Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load: a)On or about 6/27/02 through 7/3/02, the following overhead hoists had no inspections performed, broken safety latches on the lifting hooks and/ or damaged control systems: 1.BBI Rewind Area, A CM Valustar overhead hoist had a broken safety latch on the lifting hook and no inspections performed. 2.BBI Rewind Area, A Coffing 2 ton overhead hoist had no inspections performed. 3.BBI Rewind Area, A CM overhead hoist had a broken safety latch on the lifting hook and no inspections performed. 4.Finishing Room, A CM Loadstar 2 ton overhead hoist had a missing safety latch on the lifting hook and no inspections performed. 5.Pilot Beater Room, A CM Valustar overhead hoist had no inspections performed. 6.Machine Shop, A CM Loadstar overhead hoist had no inspections performed. 7.Oil Room, A Vale overhead hoist had no inspections performed and was not rated. 8.Dry End of Paper Machine #2, A Coffing 2 Ton overhead hoist had no strain relief on its control box and had no inspections performed. 9.Dry End of Paper Machine #1, A Harrington overhead hoist had no inspections performed. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Install new safety latches on the lifting hooks in accordance with ANSI B30.16-1993 Section 16-1.2.9. 2.Repair or replace the damaged control system in accordance with ANSI B30.16-1993 Section 16-2.3.3. 3.Perform inspections as required in accordance with ANSI B30.16-2.1. b)On or about 6/27/02 through 7/3/02, the following below the hook lifting devices had no load capacity markings and no inspections performed: 1.BBI Rewind Area, a yellow multipoint spreader bar was attached to a CM Valustar overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 2.BBI Rewind Area, a grey metal multipoint spreader bar was attached to a Coffing overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 3.BBI Rewind Area, a yellow multipoint spreader bar was attached to a CM overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 4.Finishing Room, a green metal spreader bar was attached to a CM Loadstar overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 5.Finishing Room, a grey metal spreader bar was attached to a chain fall type overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 6.Pilot Beater Room, a black metal spreader bar was attached to a CM Valustar overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 7.Oil Room, a Drum Grabber was attached to a Vale overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 8.Dry End of Paper Machine #2, a grey metal spreader bar was attached to a Coffing overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. 9.Dry End of Paper Machine #1, grey metal spreader bar was attached to the end of a Harrington overhead hoist. The under the hook lifting device had no rated load capacity markings and no inspections performed. Among other methods, feasible and acceptable abatement methods to correct the hazards are to:1.Mark the rated load of the lifting device on the main structure or equivalent in accordance with ANSI B30.20-1993 Section 20-1.2.1. 2.Perform inspections as required in accordance with ANSI B30.20- 4.3.1. c)On or about 6/27/02 through 7/3/02, the following monorails had no rated capacity markings on them and/ or no end stops provided on the monorails: 1.BBI Rewind Area, A monorail with a CM 1 ton overhead hoist attached had no rated load capacity markings on the rail. 2.BBI Rewind Area and Storage Area, A monorail system with hooks attached had no rated load capacity markings, and a switching point was not provided with end stops. 3.Pilot Beater Room, A monorail with a CM Valustar overhead hoist attached had no rated load capacity markings on the rail. 4.Machine Shop, A monorail with a CM Loadstar overhead hoist attached had no rated load capacity markings on the rail. 5.Oil Room, A monorail with a Vale overhead hoist attached had no rated load capacity markings on the rail. Among other methods, feasible and acceptable abatement methods to correct the hazards are to: 1.Mark the rated load on the main structure in accordance with ANSI B30.11-1980 section 11-1.1.2. d)On or about 6/27/02 through 7/3/02, the following hooks were damaged and/ or not provided with inspections: 1.BBI Rewind Area, Fabricated hooks attached to a monorail system were not inspected and some were damaged. Among other methods, feasible and acceptable abatement methods to correct the hazards are to: 1.Inspect the hooks in accordance with ANSI B30.10-1993 section 10- 1.2.1. 2.Remove or repair damaged hooks as per ANSI B30.10-1993 section 10- 1.2.3. Abatement certification must be submitted for this item.
