Text For Citation: 01 Item/Group: 003 Hazard: UNAPEQUIP
Title 21 V.S.A. Chapter 3, subchapter 5, sec. 223(a) of the Vermont Occupational Safety and Health Act. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, a smaller than recommended (6") sheave was utilized with the 5/8ths inch cable, without the manufacturers written approval, causing accelerated wear and tear and reduction of the safe operating life span of the cable. Location: Rutland Transit Center Violation: A smaller than recommended (6") sheave was in use with a 5/8ths inch cable causing accelerated wear and tear on the cable, reducing it's usable safe life span resulting in catastrophic failure and serious life threatening injuries to an employee. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to follow the recommended guidelines for sheave sizes based upon the wire/cable size.
