Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being stuck by an underhung crane or the material it was handling due to deficiencies in the running rope of the crane and the cab of the crane. The running rope of the crane had at least 7 broken wires in one strand of a 7/8" wire rope and the rear window of the cab was cracked and covered with a dirt residue, making visibility out of the window impossible. One feasible and acceptable method to correct these hazards is to replace the running rope and replace the rear window. A) LOCATION: CHARGING DECK PER THE AMERICAN NATIONAL STANDARD FOR MONORAIL SYSTEMS AND UNDERHUNG CRANES (ANSI B30.11-1973 SECTION 11-2.4.2), THE RUNNING ROPE OF THE CRANE HAD MORE THAN FOUR BROKEN WIRES IN ONE STRAND IN ONE ROPE LAY AND IT WAS NOT REPLACED. THERE WERE AT LEAST SEVEN BROKEN WIRES FOUND IN ONE STAND OF ONE LAY ON THE 7/8" WIRE ROPE USED ON THE CRANE. B) LOCATION: CHARGING DECK PER THE AMERICAN NATIONAL STANDARD FOR MONORAIL SYSTEMS AND UNDERHUNG CRANES ( ANSI B30.11-1973 SECTION 11-1.8.2), WHERE WINDOWS WERE PROVIDED, THEY DID NOT PROVIDE AMPLE VISION FOR OPERATORS. THE REAR WINDOW WAS CRACKED IN SEVERAL PLACES AND COVERED WITH A DIRTY FILM, ELIMINATING VISIBILITY FROM THE WINDOW.
