Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads from failure of a custom made lifting hook: a.Mark V Melt. Employees used a custom made lifting hook to transport 1800 lbs. starter blocks. No capacity was marked, no proof testing was performed. Among feasible abatement methods is to comply with the American National Standard for Below the hook lifting devices, ANSI B30.20 and: 1.Provide a lifting device that is designed with a safety factor of three; 2.Marked with the rated capacity. 3.Proof tested to 125of the rated capacity. ***THIS ITEM REQUIRES DOCUMENTATION OF ABATEMENT***
