Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. At the site, on or about March 12, 2012, employees operated Powered Industrial Vehicles (forklifts) to move welding machines and other items, they were not trained, evaluated and certified, exposing employees to struck by hazards.
