29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a) On or about February 6, 2012, Dukane Precast Incorporated did not prepare entry permits for permit required confined spaces in the batch plant known as sand bins number 1 and number 2. Abatement documentation is required for this item in accordance with requirements of 29 CFR 1903.(d).
