29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure: On or about November 10, 2025, at 42 West Buckeye Street, West Salem, OH 44287, the employer failed to ensure all new employees working in and around heavy equipment to include, but not limited to rubber rolling mills and calenders received affected employee lockout tagout training.
