29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about August 18, 2025, an employee constructing the new paint line were exposed to unprotected floor holes around the parts washer and the employer failed to ensure guardrails were installed near the edges of the recessed edges of the walking working surface.
