29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. On or about July 31, 2025, the employer did not ensure an engineering survey was conducted and recorded by a competent person prior to beginning demolition work on the supporting structures of the mixed-use building located at 305 E. Spring St. in St. Marys, Ohio. Employees who worked to remove and excavate sections of the foundation, a section of the south exterior wall, and/or installed jack posts were exposed to being struck and/or crushed by falling building materials in that one employee sustained fatal crushing injuries and another employee sustained facture injuries when a portion of the building's west exterior wall collapsed. No additional abatement information is needed for this item.
