29 CFR 1910.268(g)(1): The employer did not ensure the proper use of a positioning system or a personal fall arrest system while work is performed at positions more than 4 feet (1.2 m) above the ground on poles and on towers: On or about July 14, 2025, at the internet tower of the worksite an employee was utilizing a fall arrest system that was not adequately used while climbing and performing maintenance on the internet tower.
