29 CFR 1910.146(d)(9): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces: On or about June 12, 2025, and at times thereafter, the employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees for the following instances listed below: a) On or about June 12, 2025, where one (1) employee was allowed to enter and work in an underground vault; and b) On or about June 13, 2025, where two (2) employees were allowed to enter and work in an underground vault (Vault (53+60)).
