29 CFR 1926.451(g)(3): In addition to meeting the requirements of 1926.502(d), personal fall arrest systems used on scaffolds were not attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member: a) On or about April 29, 2025, on the northeast side of Tank A, an employee working from a Doka Climbing Formwork MF 240 scaffold used the guardrail system as an anchor system for his personal fall protection system. b) On or about April 29, 2025, on the northeast side of Tank A, employees engaged in jumping/lifting a Doka Climbing Formwork MF 240 scaffold platform more than 6 feet above the ground were not protected by a fall protection system.
