29 CFR 1910.147(e)(2)(i): Employees. The work area shall be checked to ensure that all employees have been safely positioned or removed. a.) On or about April 10, 2025, the employer did not ensure employees in the Maintenance Shop were safely positioned or removed from the work area at both ends of the Keystone Actuator Model MRP-088U-K-D000 prior to applying 80- 90 psi of compressed air to the air to open valve to test the movement of the pistons. As a result, an employee suffered an abdominal contusion and liver laceration when the piston shot out of the actuator and struck the employee.
