29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a.) At the worksite, located near Hidden Hammock Loop Parrish, FL 34219; On or about 3/25/25, the employer exposed employees to engulfment and fall hazards in that an extension ladder did not extend 3 ft past the landing surface for employees conducting sewer line repair operations within an excavation approximately 16ft in depth.
