29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about March 11, 2025, and at times prior; the employer exposed employees to cut/laceration hazards, in that the employer allowed an employee to operate a chain saw (Husqvarna, model 450 Rancher) without providing required personal protective equipment to include an approved face visor, and pants with saw protection.
