29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on or was near to them: a) Main Service Panel - On or about February 26, 2025, an employee was exposed to live parts and arc flash hazards when working to install 2 Current Transformers (CTs) in the Main Service Panel (240 Volts) with a red wire that had damaged insulation exposing the wire to the edge of the Main Service Panel. The Main Service Panel was not deenergized prior to starting this work.
