29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a) On or about February 26, 2025, at a wellhead in the Stark Brine Field, employees utilized a Genie Forklift equipped with two 2-point slings to remove a Blowout Preventer from the top of a wellhead without first ensuring the load was well balanced, stable, or safely arranged on the forks prior to lifting the load.
