29 CFR 1910.28(b)(3)(ii):The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a. On or about February 20, 2025, bay floor level at bay 1, 2 and 3: employees were exposed to tripping hazards while standing near or on the top of the StoneTree Concrete Fence System box to pour concrete and separate the concrete walls, in that floor holes were not covered. b.
