29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about September 10, 2024, one or more methods of machine guarding were not provided to protect the employees operating the Bridgeport mill machine when cutting gussets into aluminum tubing for safety rail production in the machine shop area of the facility located at1 High Tech Avenue, PAINESVILLE, OH 44077. With no effective guarding in place, an employee wearing gloves while operating the mill, had unrestricted access to the rotating tooling assembly and point of operation, and getting their gloved hand caught in the spindle which led to a partial finger amputation and in patient hospitalization.
