29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation, and the operator does not have a clear and direct view of the edge of the excavation. On or about February 01, 2025, inside a trench that ran south-north direction at location; no barricades or warning system was utilized or made available for heavy equipment that is used next to the trench.
