29 CFR 1910.119(d)(3)(i)(B): Information concerning the equipment of the process did not include a complete and accurate piping and instrument diagram: On or about 01/28/2025, the employer did not ensure that piping and instrument diagrams (P&IDs) were accurate. The P&IDs were not accurate as the following but not limited to: a) In P&ID R7005A, Rev. 3 (date 04/17/2023) level transmitter LT9 of the high temperature reciculator (HRT2) is not shown any connections to high temperature recirculator pumps (HTR2-P1 and HTR2- P2). b) In P&ID R7005A, Rev. 3 (date 04/17/2023), level (float) switch FLT10 of the high temperature reciculator (HRT2) is not shown any connection to high stage compressors. c) P&ID R7005 was provided at 2 different times with different revisions and date. i. P&ID R7005, Rev.7 (date 12/11/2020), High Temperature Accumulator (HTA1) was displayed but it does not exist in the field. ii. P&ID R7005 Rev. 3 (date 10/11/2021), lines and instruments associated with HTA1 have a "X" mark of them. The king valve CV-14 was displayed in these P&ID R7005 and not shown any connections to any emergency buttons or ammonia detectors.
