29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about January 24, 20245, the employer did not ensure that periodic inspections of the energy control procedure and practices for the Line 6 Extruder were conducted at least annually to verify that the procedures were adequate and utilized for maintenance and servicing tasks. Deficiencies were found in the employer's Line 6 Extruder machine specific energy control procedure and its use, had periodic inspections been done as required, inadequacies would have been identified and deviations could have been corrected. Operators performing setup tasks were exposed to an unexpected release of hazardous stored pressure, heated gases and fumes, and heated molten plastic from the extruder barrel likely to result in serious burns and other injuries. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
