29 CFR 1910.180(d)(4): Complete inspection(s) of crane(s) were not performed at intervals as generally defined in 1910.180(d)(ii) with a determination made as to whether deficiencies constituted a safety hazard: a. On or about January 16, 2025, the employer failed to ensure inspection(s) of crane(s) that included the #1 Fuchs MHL340 D mobile hydraulic loading machine, Serial Number: M340210/1700 and the Fuchs #2 MHL 340 mobile hydraulic loading machine, Serial Number: 3402100921, were periodically inspected at 1- to 12-month intervals. Employee(s) were exposed to crushing injuries while using the #1 Fuchs 340 Material handler when the structural components failed during normal operations and resulted in a catastrophic equipment failure.
