OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential chemical burns and inhalation hazards from release of ammonia, and struck by hazards from over pressure failure of ammonia refrigeration components, inspection and maintenance of the Ammonia Mechanical Refrigeration Systems is not implemented as required, exposing employees to chemical burns and inhalation hazards from release of ammonia, and struck by hazards from failed ammonia refrigeration components related to catastrophic release due to the lack of regular pressure relief valve maintenance activities: (a) Anhydrous Ammonia Refrigeration Systems: On or about January 13, 2025, the refrigeration system was not adequately maintained to protect employees from an uncontrolled release of anhydrous ammonia in that pressure relief valves (PRVs) were not being inspected or replaced every five years. This includes a Cyrus Shank PRV on the RB-2 compressor that connected the oil cooler to the oil separator was scheduled to be replaced in December 2017.
