29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Lima Tank Wash, Inc. on or about December 23, 2024 at the location 1590 Findlay Road, Lima, Ohio 45801: The employer did not ensure powered industrial truck operators were evaluated at least once every three years to certify safe operation of forklifts. Due to this condition employees working on or around forklifts were exposed to potential stuck by hazards.
