29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Perram Electric, Inc. on or about November 13, 2024, near the intersection of SR 2 and Spring Meadows Dr. Holland, OH: The employer did not ensure employee(s) operating an articulated mobile scaffold were adequatly trained to recognize and avoid hazards associated with operation of an aerial device. An employee performing work from this device without the benefit of this training was exposed to falls 13'6" or greater to the roadway below.
