29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: On or about December 2, 2024, an employee was exposed to amputation hazards, while performing service and maintenance operations on Conveyor 8 in the Basement, Room 1. The employer did not ensure that the employer's authorized employees were aware and informed of the on-site employer's lockout or tagout procedures for onsite equipment.
