29 CFR 1910.147(d)(4)(i) as required by 29 CFR 1960.8(b): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or before December 3, 2024, the employer did not ensure employees affixed an energy isolating device (lock) on the electrical disconnect of the Building 301, Electric Propulsion Laboratory, Fabrication Shop, Continental Machines, Inc, Do-All Band Saw, Model 3613-20, NASA Asset No. 277623, during service and maintenance activities such as changing the band saw blade (setup).
