29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the converting and envelope machines for the BOE model number - 615, serial number - 4638: The guard for the converting section was in an up/open position during operation exposing employees to amputation and entanglement hazards from rotating parts. b. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the energized converting and envelope machines for the BOE model number - 601, serial number - 4637: The guard for the converting section was in an up/open position during operation exposing employees to amputation and entanglement hazards from rotating parts. c. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the energized converting and envelope machines for the BOE model number - 620, serial number - 5504: The guard for the converting section was in an up/open position during operation exposing employees to amputation and entanglement hazards from rotating parts. d. Additionally, the guard for other rollers near the feed section was lifted and not secured, creating an opening for employees to reach into the point of operation, exposing employees to ingoing nip point hazard. m.
