29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about November 20, 2024, an employee performing servicing and maintenance activities on equipment was exposed to hazardous stored energy, including but not limited to, electrical and gravity, and the energy control procedures did not include specific procedural steps for testing the equipment to verify the effectiveness of the lockout devices: The machine specific written procedures for the: a. Mill 1 did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled. b. 48" Mill did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled. c. Stolarczyk Re-Coiler did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled. d. Schlebach CTL did not include steps for testing and verifying that all electrical energy sources had been dissipated and effectively controlled. e. Side Shear did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled. f. Back Shear did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled. g. Mill 5 did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled. h. The Jorns Normaline folder lockout procedures did not include step for testing and verifying that electrical and hydraulic energy sources had been dissipated and effectively controlled. i. The Thalman folder lockout procedures did not include step for testing and verifying that electrical and hydraulic energy sources had been dissipated and effectively controlled. j. FJ-10 Mill lockout procedures did not include step for testing and verifying that electrical and pneumatic energy sources had been dissipated and effectively controlled.
