29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about November 19, 2024, an employee performing servicing and maintenance activities on equipment was exposed to amputation and entanglement hazards and the employer did not ensure the machine specific energy control procedures included specific requirements for testing a machine or equipment and verify the effectiveness of lockout devices, tagout devices and other energy control measures, including but not limited to the following energy control procedures: a. the energy isolation procedures for the Trial Size Machine did not include specific procedures on how to verify each energy source, including thermal and pressure energy from the liquid nitrogen, was dissipated. b. the energy isolation procedures for the Roller Mill Machine did not include specific procedures on how to verify each energy source, including electrical, was controlled, dissipated and isolated. c. the energy isolation procedures for the Toyo Packaging Machine did not include specific procedures on how to verify each electrical energy source was controlled, dissipated and isolated and each pneumatic energy source was identified.
