29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a. While accessing a second story 6:12 pitched roof, at a temporary worksite located at 1008 North Central Avenue in Flagler Beach, Florida: On November 14, 2024, the employer exposed employees to fall hazards, in that an unsecured non-self-supporting extension type ladder was placed on the slant/peak of the first floor roof and was utilized by employees to enter and exit the second story roof, while conducting roofing work. b. While accessing a second story 6:12 pitched roof, at a temporary worksite located at 1008 North Central Avenue in Flagler Beach, Florida: On November 14, 2024, the employer exposed employees to fall hazards, in that an unsecured fiberglass self-supporting A-Frame type ladder was placed on the slanted first floor roof and was utilized by employees to enter and exit the second story roof, while conducting roofing work.
