29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about October 21, 2024, the employer directed a concrete pour using a Schwing 36-meter concrete pump truck near an overhead electrical transmission line without using a spotter to maintain a safe operating distance from the high voltage overhead electrical hazard of 14.4KV.
