Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Truck mounted winches, west side parking lot, a complete inspection of the hoist was not performed at intervals as generally defined in ANSI B30.7 1971-72.1.1.b.2 depending upon its activity, severity of service and environment or as specifically indicated below: 1) The notches on the winch clutch locking system was worn 2) The clutch pull rod was bent. 3) The dogs in the clutch engaging system was worn. 4) A wad of grease was on a section of the dogs when engaged. A feasible and useful method to correct this hazard, among other methods is to replace the dogs, notch section and install newer rods to prevent their bending.
