29 CFR 1910.28(b)(3)(i): Holes. The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: (b)(3)(i)(B) Guardrail systems; (b)(3)(i)(C) Travel restraint systems; or (b)(3)(i)(D) Personal fall arrest systems. a) On or about 09/07/2024, the employer did not ensure that prior to removing the cover for the 9 foot rotary 4 hole located in the cotton room, was protected by one or more of the following: a guardrail system, travel restraint system, or personal fall arrest systems to prevent exposure to a fall hazard for all employees working in the area.
