29 CFR 1910.119(j)(5): Employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner when necessary means were taken to assure safe operation: Establishment: Employer did not correct deficiencies in the following ammonia refrigeration equipment that were outside acceptable limits as per employer's process safety information in a timely manner to assure safe operation: a) On or about September 3, 2024, a drain valve and drain plug for a recirculating pump for the low pressure receiver (LPR) vessel leaked approximately 721 pounds of ammonia refrigerant when the drain valve and drain plug were not effectively closed. b) On or about November 17, 2024, a purge line pipe for the LPR began leaking ammonia refrigerant when the purge line pipe failed.
