29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). Location - Platform on roof of Fabric Filter #1 Building, #1 Deflation Fan. On or about 9/3/2024, the baghouse dampers connected to the #1 deflation fan were not physically locked out and tagged out with energy isolating devices prior to performing preventive maintenance on the #1 deflation fan. Stored energy within the dampers was not relieved, disconnected, restrained, and otherwise rendered safe.
