29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: On or about July 24, 2024, at a workplace located at 500 County Road S, New London, Wisconsin, in the Hog Room; each employee engaged in a servicing and/or maintenance tasks did not affix an lockout device, i.e. lock, to each energy disconnect prior to engaging in a jam clearing activity on the WSM Dry Hog.
