29 CFR 1910.140(c)(6): The employer did not ensure that a competent person or qualified person inspected each knot in a lanyard or vertical lifeline to ensure that it meet the requirements of paragraphs 29 CFR 1910.140(c)(4) and (5) before any employee used the lanyard or lifeline: a) Facility Wide - On or about August 19, 2024, employees were required to use a personal fall arrest system when operating order pickers and were exposed to a fall hazard of approximately twenty-three (23) feet because a knot in the lanyard being used had not been inspected by a competent person. Certification Not Required.
