29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized: On or about August 8, 2024, at 20345 Golf Course Road, Boca Raton, Florida, the employer failed to render a circuit inoperable and place a tag to identify the circuit being worked on prior to allowing a worker to install a power pack.
