29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. a. On or about July 15, 2024, the employer did not evaluate, at least once every three years, the knowledge and skills of the employees who operated the powered industrial trucks in the facility including, but not limited to, the Toyota electric forklift truck, model 7FBEU20, serial no. 17704, the Clark electrical lift, model TM15S, serial no. TM247-0014-9340FB, the Hyster lift truck model N35ZR-165, serial no. D470N02183F, and the Hyster lift truck model S80FT, serial no. H004V01569K.
