29 CFR 1910.147(a)(3)(i): The employer failed to establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees. a) Throughout the warehouse - On or about July 09 2024, and times prior, the employer exposed employees to electrical shock and struck by hazards in that, employer failed to establish and provide a LOTO program for the baler and power industrial trucks.
