29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. The employer did not annually certify that operating procedures were current and accurate. a) On or about 6/27/2024, at Stonewall Gas Plant, the employer did not ensure that the "Inlet gas system procedures" were reviewed as often as necessary to assure that they reflect current operating practice. The following items are listed in the procedure are inaccurate but not limited to: 1. "PSV-6005 - Pressure relief valve sized for fire, set at 1200 psig." was listed in page 9. 2. "LAH-6007 - High level alarm, set at 100%" was listed in page 9. 3. "PAH-6074 - High pressure alarm, set at 1200 psig." was listed in page 9. 4. "PAHH-6004 - High-high pressure alarm, set at 1200 psig." was listed in page 9 5. "FV-6003A/B - Control valve is positioned based on a low select control scheme between pressure (PY-6004) at the slug catcher and flow (FY-6003) via the plant inlet meter FT-01" was listed in page 9 and 10. b) On or about 6/27/2024, at Stonewall Gas Plant, the employer did not ensure that the "Cryogenic Processing System procedure" were reviewed as often as necessary to assure that they reflect current operating practice. The following items are listed in the procedure are inaccurate. 1. In the "Alarms" table, the employer listed high level alarm LAH-404 with set point of 75%. 2. In the "Alarms" table, the employer listed low level alarm LAL-404 with set point of 25%. 3. In the "Control Loops and Analog Indication" table, the employer listed level indication control LIC-404 with a setpoint of RH at 100%.
