On or about June 5, 2024, and at times prior, the employer exposed employees to struck-by and crushing hazards, in that, training on fork and attachment adaptation, operation and limitations specific to the bucket shovel attachments used on the JCB Loadall and JLG telehandler was not provided to operators.
