Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) 7727 U.S. 160, ALAMOSA, CO 81101: On or about May 15, 2024, and at times prior, the employer did not ensure the John Deere tractor and power take off ("PTO") were de-energized and power to the 2020 Massey Ferguson 2270 Large Square Baler was isolated, as required by this standard resulting in an employee suffering fatal injuries while performing maintenance and lubrication of the 2020 Massey Ferguson 2270 Large Square Baler. Employees were routinely performing these operations without isolating and de-energizing the tractor and PTO.
