29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) Jobsite located at or near 6523 Lansdowne Ave, Philadelphia, Pennsylvania- On or about May 8, 2024, the employer did not ensure the equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. The employer did not examine the stud welder, Midwest Fasteners, Inc., Model: Talon, Serial Number: TAL-1217-235 prior to use and allowed employees to use with defects such as, but not limited cables of the welder had numerous cuts in multiple areas of exposed wires and a screw on the weld gun was missing, thereby exposing employees to electrical hazards. b) Jobsite located at or near 6523 Lansdowne Ave, Philadelphia, Pennsylvania- On or about May 8, 2024, the employer did not ensure the equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. The employer did not examine a AWG 14-gauge extension cable prior to use and allowed employees to use with defective ground and exposed insulation, thereby exposing employees to electrical hazards.
