29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about April 10, 2024, employees engaged in paper printing activities in the Northside area of Taylor Communications located at 9503 F Street, Omaha, Nebraska were exposed to caught-in and crushing hazards in that the employees did not isolate pneumatic and electrical hazardous energy by affixing a lock and/or tag prior to clearing paper jams on the EVTEC Sheeter.
