29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about April 29, 2024, the employer failed to ensure that an employee operating the Crown RC5535 Counterbalance Forklift was competent to operate the powered industrial truck safely by successfully completing powered industrial truck training and evaluation. (b) On or about April 29, 2024, the employer failed to ensure employee(s) operating the Toyota WP3035 Electric Pallet Jack and the Crown WP3035 Electric Pallet Jack were competent to operate the powered industrial truck safely by successfully completing powered industrial truck training and evaluation.
