29 CFR 1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices to verify that conditions in a permit space were acceptable for entry throughout the duration of an authorized entry: a) On or about 04/15/2024 at the site located at 3000 Milton Ave, Solvay, NY 13209: Employees entered the Lindberg Kiln without testing for atmospheric hazards throughout the duration of the entry, exposing employees to asphyxiation hazards due oxygen deficient environment.
