On or about 4/15/2024 the employer failed to ensure the Machine Shop Metal Working Fluid Dispensing system was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party. b) Frazer & Jones, LLC, 3000 Milton Avenue, Solvay, New York, 13209.
